This really seems bizarre - you're saying that it shows up in dmesg
as /dev/sda, but 'ls /dev/sda' shows 'No such file or directory' ?!

Please report the *exact* dmesg and ls output here.

If you can't find it, you have worse problems than formatting.  One
thing at a time.
