There /is/ no file `/usr/sbin/add-shell' in sarge's debianutils:
http://packages.debian.org/cgi-bin/search_contents.pl?word=%2Fusr%2Fsbin%2Fadd-shell&searchmode=searchfiles&case=insensitive&version=stable&arch=i386

Which leads to some obvious questions you'll want to consider.
