The answer was to place the line
<< /Duplex true >> setpagedevice
near the start of the Postscript file.
The C code I use to write the very minimal Postscript header is now
fprintf(printer, "%! PRINTPS.CPP PostScript OUTPUT\n");
fprintf(printer, "<< /Duplex true >> setpagedevice\n");
fprintf(printer, "/cour /Courier findfont %d scalefont def\n",
linesize);
fprintf(printer, "cour setfont gsave\n");
instead of
fprintf(printer, "%! PRINTPS.CPP PostScript OUTPUT\n");
fprintf(printer, "/cour /Courier findfont %d scalefont def\n",
linesize);
fprintf(printer, "cour setfont gsave\n");
