TBT, I haven't run a distros kernel version in ages, always building
from kernel.org sources. I've not encountered such an error, ever.
One thing my scripts do that you may not be doing, is they are
renameing all potential duplicate paths or files with the
extension .old appended, so that a repeat install is both out of the
picture AND can be recovered with a few rm's, & mv's. I only do that
for one generation as they also remove any of the version.number.old
stuff they find ahead of the renameing done.
In other words, whats in my /lib/modules/uname -r`/kernel directory is
_only_ what was installed by this incarnation of the make
modules_install. I suspect theres something laying about in that
tree that probably should go away.
Similarly, I don't use an initrd as I'm not booting from a filesystem
thats not built into my kernels.
My scripts do the same bit of renameing vis-a-vis the System.map
files. I do NOT do a make install to put in the vmlinuz file, but do
my own copying there so the version number is appended, ditto for the
System.map which is actually a softlink to System.map-2.6.x.x like
everything else.
This machine, being my playtoy, has a grub menu choice of nearly 20
kernels I could boot it from, not all of which work of course. ISTR
the last time I tried to boot dos from grub, something missfired. I
have freedos available on floppies for that anyway :-)
