Try this as root.
# dpkg-reconfigure xserver-xfree86
When asked if you want to "use kernel framebuffer device interface?"
answer No. Otherwise keep your existing settings. (This seems to be
the magic solution for 90% of such problems.)
Your settings for horizontal sync and vertical refresh could probably
be optimised, if you can find out exactly what they should be. It
might make a small difference to performance for games.
