What's customized in "Debian-based" distros?

I've heard people advocate the newbie-kindness of distros like Ubuntuand Mepis.

If ultimately they use the same set of packages, then what's differentat a technical level between these and plain old Debian? Do theydiffere merely in the set of initially installed packages and what theydo under /etc ?
