Depends on your ISP.

No you can define just a valid ip address and expect

Yes the nameservers generally the ones that your ISP
gives you, at least you will configure BIND9

Finally I think you must specify if you have an static
ip address or a dinamyc one.
The whole configuration depends on how you get your
ip.
