FWIW here's what I had to add to /etc/modules to get my mouse to work.
ohci-hcd
mousedev
psmouse
You may need to substitute uhci-hcd or ehci-hcd instead of ohci-hcd, depending on your hardware. I saw Bluetooth loading *HCI devices, but not plain USB in your output. I could have missed something.
