looks like a hardware problem. are you using a SCA hot swap backplane?
or a SCA->68pin adapter?

I have yanked drives out of a software raid 1 system(no LVM) without
any problems. I ran my tests on an intel l440GX+ with onboard
AHA-2940U2W controller connected to a 2-disk SCA hotswap backplane
which had 2x18GB SCA disks using a LVD scsi cable.

maybe you need a better SCSI card.

but the kernel messages look to me like the hardware is at fault rather
then the software.
