What do you mean? 
1. How to patch a standard kernel with the debian freeswan patch, or
2. how to patch an already patched freeswan kernel with additional 3rd party 
patches (X.509) ?

For 2.: it's not necessary, because the freeswan in debian contains already 
3rd party patches.

For 1.:
- - install the kernel sources and the freeswan patch from the debian servers
- - cd /usr/src
- - unpack the kernel sources
- - make a symlink 'linux' to the kernel source dir.
- - cd linux
- - export PATCH_THE_KERNEL=YES
- - make-kpkg -bzimage -config menuconfig kernel-image
when finished:
- - cd ..
- - install the kernel-image package

and you are done.
