Some do, some don't, I think. I have built a VGA-to-Mac adaptor and
did indeed need to insert sync on green. On the other hand I have a
pinout list which includes a D15 Mac monitor connector with separate
sync.

If you buy an adaptor it SHOULD (yeah, that word) insert sync on green.

I half-remember the RGB levels being different as well, but I've lost
the circuit diagram now. Again, it is the job of a boughten adaptor to
sort that out.
