Wow, this are a loooot of fields - I count 90.
In abook, you only have 15 fields:

name ("last, first" or "first last"?)
email (can contain up to 4 addresses)
address
address2
city
state
zip
country
phone
workphone
fax
mobile
nick
url
notes

You realize all other data will be ignored (i.e. lost).

Do you want to use "Home", "Business" or "Other" addresses from the
cvs file in abook?
Oh, and "notes" can be only one line.
