I have encountered something similar once with reiserfs.  There is a
patch somewhere that supposedly fixes this, but I don't think it has
been rolled into the standard kernel yet.  Search the reiserfs list for
something like "ordered writes".  I believe the patch is by Chris
Mason.  It basically ensures that data is written to the disk before the
metadata is written.
