I have the same card and it works.
I'm using a stock Woody Xfree-4.1.0 with mgadrivers-2.0 downloaded from
here:

Do you use the same?
Are you sure your XF86Config-4 is correct?
Try to disable framebuffer and verify you do not have a duplicated entry
for your mouse (I had those problems the first time):

Section "Device"
        Identifier      "Generic Video Card"
        Driver          "mga"
        #Option         "UseFBDev"              "true"

Section "ServerLayout"
        Identifier      "Default Layout"
        Screen          "Default Screen"
        InputDevice     "Generic Keyboard"
        InputDevice     "Configured Mouse"
        #InputDevice    "Generic Mouse

I got no problem with provided install.sh.

Do not even try, it's useless with this card.

I don't use dualhead so I can't help about that.
