it says  = localhost:localhost

Sorry, yes.  That's what I think is happening now.

I'm using Sylpheed to read the mail.  Originally, I tried to set things up
so that exim fetched the mail and Sylpheed read it locally, but I got into
a mess whereby outgoing messages were deferred ( Connection refused by
[127.0.0.1] - it seemed as though I was trying to send messages back to
myself.)

My original motivation was to circumvent the problem that because Sylpheed
is not threaded you can often wait quite a long time before you get
control of the program back when you send or receive messages. Having exim
do all the work "transparently" seemed to be the ideal solution.  But it's
hard to match the documentation to my needs.
