The solution I have used for my usb floppy is very nasty but it works. I
took a blank disk and created an image from it with "dd if=/dev/sda
of=blank_floppy.img" and then to format a new disk I can just "dd
if=blank_floppy.img of=/dev/sda" like I say though, this is ugly and
there is possibly a better way to do it, I just don't know what that is.

I don't know about the booting issue as I've never tried before. Do the
debian boot-floppies work?
