It looks like you're booting with init=/bin/sh or that you
linked /sbin/init to /bin/sh, because that is exactly what would
happen. Don't do that.

Sounds like /var and /usr are not mounted. Which makes sense
if you boot with init=/bin/sh since the kernel will just spawn
a shell after boot and that's it, no other initialization.
