I'm not sure if it's documented in the 1.3.x manual, but the '*'
represents threading that Mutt assumes based on the subject header
because other threading headers were not available. You can turn it off
with something called 'strict_threads' or similar.
As for the '?', that represents a gap in the threading from messages
either missing or not being shown in the current limit pattern.
