This is what my /etc/modutils/bttv looks like:

and this is the relevant part of my lsmod output:

try:
modprobe bttv card=1; modprobe tuner type=0

Since I have a similar card as you this should work. I'm assuming
of course that you have the bttv related things compiled as
modules.
