You should choose the driver for the card in the device section (at
least IMHO) and put it in here.

Are you sure the last "Screen" section is not overwriting the previous
ones?  Try neaming the screens sections and using them in the
severlayout as below.

You should have a serverlayout section as below as well (IMHO)

Section "ServerLayout"
        Identifier      "Default Layout"
        Screen          "Screen"
        InputDevice     "Keyboard"
        InputDevice     "Pointer"
EndSection
