This is what cat /proc/pci gives me for my PCI modem.

Communication controller: PCI device 151f:0000 (TOPIC SEMICONDUCTOR Corp) 
(rev 0).
      IRQ 5.
      I/O at 0xec00 [0xec07].
  Bus  1, device   0, function  0:

If you are not getting I/O address there, maybe your modem is so called 
Winmodem?
