I do:
devpts on /dev/pts type devpts (rw,gid=5,mode=620)

The the normal xterms or gnome-terminal's work, they are using the /dev/pts/ 
as their controlling tty. It's the applications that aren't being run from a 
term that aren't calling x11-ssh-askpass. If I run tkcvs in the foreground of 
an xterm the passphrase is asked within the tty of the xterm, /dev/pts/0, if I 
call it up via the menu's it errors out.

I suppose if I have to I can start all of my apps from in the foreground of a 
pile of xterms . . . ick!
