I got the exact same error message after I upgraded libdb2 or something
some time ago. The fix was to shutdown exim and remove files (retry and
wait-remote_smtp were present on my system) from /var/spool/exim/db.
After that I don't remember seeing any errors.

Use with caution as I don't know what side-effects that removal might
have. It worked for me, but your mileage may vary.
