you should not have to install ybin yourself, if you used recent boot
floppies its in the base system (in package yaboot) and yes
menu_ofboot.b is fine in 0.16, you just have to adjust the
OpenFirmware paths to match your system (usually just change the X to
the right partition numbers.

to mount an HFS floppy run:

mount -t hfs /dev/fd0 /floppy

if for some reason you don't have kmod (from your above message) run 

insmod hfs
