Here is what I have in my /etc/X11/XF86Config:

Section "Pointer"
    Protocol    "MouseManPlusPS/2"
    Device      "/dev/mouse"
    ZAxisMapping 4 5

Make sure that /dev/mouse points to the correct device (/dev/psaux for
me since mine is a PS/2 mouse).
