I had something like that when I deleted by mistake the relevant line from 
/etc/inetd.conf.
With exim it is something like:

[02:16:38 /tmp]$ grep smtp /etc/inetd.conf
smtp                    stream  tcp     nowait  mail    /usr/sbin/exim exim -bs
[02:16:45 /tmp]$
