This may be your problem.
Most monitors nowadays accept a range of hsync frequencies, e.g. 31-50.
I've had similar problems with a 'bi-sync' monitor (NEC MultiSync 2A) which
is specced as working on two discrete, fixed frequencies (31.5, 35.2).
In order to get the modes I expect, I have to specify these in xf86config
as narrow ranges, rather than as discrete values (e.g., 31.4-31.6,
35.1-35.3). I suspect that the arithmetic in calculating scan rates
required for the various video modes produces scan rates that are close
enough to the spec value, but which is discarded because it is not an exact
match. Try specifying hsync as, e.g., '31.4-31.6'.
