/etc/ppp.options_out is used by the 'pon' command and by init.d.
/etc/ppp/options.ttyXX is read by pppd when it is called with ttyXX as an
argument. It usually is not needed.

No. You don't need to mess with /etc/ppp/options.ttyXX files unless you
run pppd on several different ports and want to use different options on
each.

The format is quite valid for a pppd option file. Pppd ignores newlines
and accepts comments (in files, not on the command line).
