Do you have /dev/ippp* ?
You can do a "/dev/MAKEDEV isdn-tty isdn-io isdn-ippp" anyway, it won't do
any harm.
And do you have a line "pppbind" in /etc/isdn/device.ippp0 ?

Did you set huptimeout in /etc/isdn/device.ippp0 ?
