mas vamos a algumas perguntas:

vc habilitou as portas seriais extras no kernel?
CONFIG_SERIAL=y
CONFIG_SERIAL_EXTENDED=y
CONFIG_SERIAL_MANY_PORTS=y

qual a saída de
# lspci -v

que versão de kernel vc está utilizando?

eu tb tenho um hardmodem PCI; a saída lspci no meu caso é:

no meu /etc/serial.conf eu tenho a seguinte configuração:

eu uso o devfs; se vc não usa, mude o dispositivo (/dev/ttyS4) ou algo
do tipo.

depois re-execute o setserial
#/etc/init.d/setserial restart
