It's in the former psfonts (or txfonts) package, now included in tetex-bin, afaik. I have not installed any extra fonts.

- In your test document, you only use text fonts of \familydefault phv,

  version use postscript fonts with dvipdfm.

Your minimal document also showed the problem.

- Which map entries are in your /etc/texmf/dvipdfm/config? If you didn't
  change the original file, is should be at the end and only say

% Font map files; maintained by updmap(1)
f dvipdfm.map
  If you changed it, please give us the whole file.

I had not changed it, but it was different from what you have.
I think this was the problem. It may have been caused by an old dvipdfm package that used to be installed. I have completely purged tetex-base and reinstalled it. Now the config file looks as you say and the problem is completely gone! (Including all warnings...)

In other words: it was a local config problem. I guess you can close the bug now. :)
