I reproduce it in an old version of passivetex (1.21-1) and that's why I
closed it since I was pretty sure it is related.  I have no problem in
compiling in either .dvi or .pdf using 'xmltex break-docbook.fo'

However, I use a powerpc and everything goes smooth.  I just try it
here, at job, on a x86 with the same version of software and it breaks.
Could the bug be in tetex?  CC the tetex maintainers about this.

I also included a short version of break-docbook.fo, with most of the
unnecessary tag remove, to better isolate the problem.  However, I think
the bug is more related to either passivetex or tetex, not xmltex.  Let
at least change it to passivetex.
