Bug#465104: openssh-server: mention /etc/ssh/sshd_not_to_be_run in /etc/ssh/sshd_config

This file is a hangover from ancient versions of the Debian OpenSSH
package, and the only reason it's still supported is to avoid breaking
old installations; it certainly isn't intended for this use. I recommend
instead disabling the ssh init script (although you will also have to
arrange that /var/run/sshd is created if /var/run is cleared at boot);
the rune is documented down near the end of
/usr/share/doc/openssh-server/README.Debian.gz.

If we need better packaged support for running sshd from inetd, I would
rather that be a new facility rather than piggybacking on this obsolete
one.
