Bug#465104: openssh-server: mention /etc/ssh/sshd_not_to_be_run in /etc/ssh/sshd_config

In order to run openssh from inetd, a special file needs to be created:

  /etc/ssh/sshd_not_to_be_run

While making the configuration changes, it would be helpful if this
file were announced at the beginnig of file

  /etc/ssh/sshd_config
