A mdbook plugin that provides a table of contents for each page. Uses
css/js from mdBook-pagetoc.

This is a little bit confusing because there are two pagetoc projects
out here, this one which is a plugin for the html output, and another
one, which provides the JS/CSS files:

I guess both would need to be packaged somehow.
