A preprocessor for mdbook to add inline Table of Contents support.

It turns this marker:

<!-- toc -->

into a Table of Contents based on headings of the chapter following
the marker.

It also supports changing the marker to an arbitrary string to support
markers like GitLab's [_TOC_]

Not to be confused with mdbook-pagetoc, which adds a floating sidebar
instead of an inline table.
