I used to know the code pretty well, but it's changed a lot
since then. My guess is that something got deleted before
you were actually done with it. A standard UNIX filesystem
is much better about holding onto files in use during unlink
due to the separation of directory entries and inode data.
However, HFS and HFS+ store all that information together,
so it's hard to unlink a file while it is open.
