I was able to use xev on my pmac 3400c to determine the keycodes for
F11 and F12.

On this system, adding the following lines to /etc/sysctl.conf maps 
F11 to the "middle" button and F12 to the "right" button.

notice mouse_button_emulation is set to 1

dev/mac_hid/mouse_button_emulation=1
dev/mac_hid/mouse_button2_keycode=87
dev/mac_hid/mouse_button3_keycode=88
