Regarding point 2, I don't think KDE users should regenerate their
icons cache from time to time. I have been using KDE on Debian for
more than 8 years, and this is the first time I experience such an

I'm afraid this may prove tricky to reproduce. I tried to test
Miroslaw's workaround on a different account on this machine. Before I
removed the file, I noticed the icons were already fine on that
account. Yet, the user logs in about daily. It's less used than my own
account, but I don't see why it wouldn't be affected too.
Oh, there's a pretty easy and likely explanation. The other user most
likely never used LibreOffice (or even explored the Office menu).
