The main difference is the output format:
netstat-nat uses a table output format similar to netstat, which in my
eyes is more human readable than the output of conntrack.
Additionally, netstat-nat can resolve IP addresses and port-numbers.
conntrack, the official tool from netfilter.org, provides a full
interface to connection tracking and has some other features like xml
output, while netstat-nat can only display the most important
information about tracked connections. See the man pages for the
details.
A short look to the popcon statistics [1,2] shows, that netstat-nat
enjoys about the same popularity as the some years younger conntrack.
