Probably problems with the source file.

Not an error, just a warning.

That's a bug somewhere in the TeX-generating code, most likely.
Perhaps you can place the SGML file somewhere where it can be
downloaded for checking.  Also, please let us know which version of
the SGML tools packages you are using.
