You have to do it by hand and normaly patch is pretty good in merging.

Also you should send your patches upstream so the maintainer can
include your bugfixes. If the upstream author doesn't like debian he
might not include the debian dir, but that should allways patch
cleanly. Everything else should be minimal changes unless the upstream
author is unwilling to accept you changes.

As you see its a good way to be friend with the upsteam author. :)
