That's up to you.

If you prefer to send it to the Debian maintainer you should file
a whishlist bug and attach the patch.

Please make sure that you document the patch.  Keep in mind that
the maintainer has to understand it and also the upstream author.

I don't know about -U 3, I prefer "diff -Nur d.orig d".
