Yes, either by modifying the filename's lines in the patch and
cat patch|patch -p0, or by using cvs and simply importing new
versions (cf. howto in cvs-buildpackage)

This is a proper way afaik.

Mostly, yes. Often it's more difficult, sometimes it's easier (only
copy the debian/* files, edit changelog and you're done).
