apt-cacher and debian-live --debian-installer enabled (Re: d-l and installer, woes)

If you pass call lh_config with --debian-installer enabled, then
lh_build will later retrieve a kernel (vmlinuz) and initrd.gz, as well
as a number of .udebs. Then, you can boot the generated image with
"install". The CD also has a live "session" capability (though there
may be a way to build an installer-only image).
apt-cacher previously didn't allow retrieving the kernel components so
I supplied a patch to allow it to be used while building an installer
image.
