I have almost the same behavior on a toshiba tecra 8000 PII 400 Mhz (old but
reliable)
kernel== 2.6.2 from debian, ram disk version for i686 (it did not happen
before with 2.4.22)
X==4.3
Xserve==neomagic
kde==3.1.4
display man also kdm, happened with gdm as well.

I hope I have added more clues to solve both problems
