So, what do you get with "cardctl ident"?
And what is written in /etc/pcmcia/config about your card?

Mine says:
card "3Com ... Fast EtherLink XL"
  manfid 0x0101, 0x5157
  bind "3c575_cb"

and cardctl ident gives me:
manfid: 0x0101, 0x5157
for mycard

See, it is very simple...
card "Card Name"
  manfid xxx, xxx    (the same as cardctl output)
  bind "driver"

There are other things you can put instead of manfid,
but this is the most straightforward method and easiest to do.
manfid, AFAIK, is a number distinct for each card.
