Check two things:

1) Your PCMCIA network connection is configured in
   /etc/pcmcia/network.opts and NOT /etc/init.d/network.

2) There exist symlinks similar to the following:

    /etc/rc0.d/K87pcmcia -> ../init.d/pcmcia
    /etc/rc1.d/K87pcmcia -> ../init.d/pcmcia
    /etc/rc2.d/S13pcmcia -> ../init.d/pcmcia
    /etc/rc3.d/S13pcmcia -> ../init.d/pcmcia
    /etc/rc4.d/S13pcmcia -> ../init.d/pcmcia
    /etc/rc5.d/S13pcmcia -> ../init.d/pcmcia
    /etc/rc6.d/K87pcmcia -> ../init.d/pcmcia
