I would like to declare this bug a hardware problem and suggest that it be closed as such: The Thinkpad that was generating this problem suffered a motherboard meltdown. With the replacement motherboard I am no longer seeing this problem, with the same kernel and mouse. I suspect that a slightly flaky USB subsystem was just one of the symptoms of a lemon motherboard.
