I have root on lvm and tried to install some Debian kernel 2.6.6. This did not 
work, as mkinitrd failed miserably. After some poking, my understanding is that 
the awk-parsing in lvmdetect2() inside mkinitrd depends on particular settings 
of lvm.conf.  Specifically, I had

    command_names = 1

in /etc/lvm/lvm.conf. This seemed to be incompatible with mkinitrd.

Workaround: Setting this back to the default 0. After that, the kernel 
installation succeeded.
